CHECKLIST OF REQUIREMENTS

FIRST ENVELOPE: ELIGIBILITY AND TECHNICAL REQUIREMENTS

  1. Eligibility Documents
    1. Valid Registration Certificate from: the Securities and Exchange Commission (SEC) for corporation, Department of Trade and Industry (DTI) for sole proprietorship, or Cooperative Development Authority (CDA) for cooperatives, or any proof of such registration.
    2. Valid Mayor’s permit issued by the city or municipality where the principal place of business of the prospective bidder is located;
    3. Statement of all its ongoing and completed government and private contracts within the LAST 5 YEARS, including contracts awarded but not yet started, if any;
    4. Statement of the Bidder’s Single Largest Completed Contract similar to the contract to be bid, in accordance with ITB Clause 5.4;
    5. Audited financial statements (2019), showing, among others, the prospective total and current assets and liabilities, stamped “received” by the Bureau of Internal Revenue (BIR) or its duly accredited and authorized institutions.
    6. Tax Clearance per Executive Order 398, Series of 2005, as finally reviewed and approved by the Bureau of Internal Revenue (BIR) valid at the time of bid opening.
    7. 2020 Income Tax Returns and business tax returns for the month of December 2020 or 4th Quarter of 2020 filed and paid through the BIR Electronic Filing and Payment System (eFPS).
    8. NFCC computation in accordance with ITB Clause 5.5;
    9. Valid PhilGEPS Certificate of Registration and Membership including Annex “A” (under Platinum category).
    10. 10. Valid/Current year permit/license from NTC or from the government regulating body/agency.
  2. Technical Documents
    1. Bid security in accordance with BDS clause 18.1, as amended which shall be limited to Bid Securing Declaration or any of the following:
      1. 2% of the ABC for Cash
      2. 2% of the ABC for MC, CC, Bank Guarantee or Irrevocable Letter of Credit issued by a Universal or Commercial Bank
      3. 5% of the ABC for Surety Bond (The Surety Bond must be accompanied with a certification issued by the Insurance Commission and must unequivocally state that the issuing Surety or Insurance company is specifically authorized to issue Surety bonds callable on demand. It is also required that the certification must be project specific stating the details of the issued surety bond which includes the project name and bond amount.
    2. Project Requirements, which shall include the following:
      1. Organizational chart for the contract to be bid
      2. List of contractor’s personnel to be assigned to the contract to be bid, with their complete qualification and experience data.
    3. Omnibus Sworn statement using the prescribed form in Section XI. Bidding Forms.
