CHECKLIST OF REQUIREMENTS

 

FIRST ENVELOPE: ELIGIBILITY AND TECHNICAL COMPONENT
    1. Eligibility Documents
      1. Registration certificate from: the Securities and Exchange Commission (SEC) for corporation, Department of Trade and Industry (DTI) for sole proprietorship, or Cooperative Development Authority (CDA) for cooperatives, or any proof of such registration.
      2. Valid Mayor’s permit issued by the city or municipality where the principal place of business of the prospective bidder is located;
      3. Statement of all its ongoing and completed government and private contracts within the LAST 10 YEARS, including contracts awarded but not yet started, if any;
      4. Statement of the Bidder’s Single Largest Completed Contract similar to the contract to be bid, in accordance with ITB Clause 5.4;
      5. Audited financial statements, stamped “received” by the Bureau of Internal Revenue (BIR) or its duly accredited and authorized institutions, for the preceding calendar year (2017).
      6. Tax Clearance per Executive Order 398, Series of 2005, as finally reviewed and approved by the Bureau of Internal Revenue (BIR) valid at the time of bid opening.
      7. Latest income and business tax returns filed and paid through the BIR Electronic Filing and Payment System (eFPS) (within the last 6 months); a. Quarterly ITR of 2018(First QTR BIR Form 1701Q or 1702Q) b. Quarterly VAT Return of 2018 for VAT Registered (First QTR BIR Form 2550Q) c. Monthly VAT Declaration (January to May 2018) for VAT Registered (BIR Form 2550 M).
      8. NFCC computation in accordance with ITB Clause 5.5;
      9. Valid PhilGEPS Certificate of Registration and Membership including Annex “A” (under Platinum category)
      10. Valid Well Drilling Certificate of Registration issued by the National Water Resources Board
      11. Valid PCAB LICENSE

    1. Technical Documents
        1. Bid security in accordance with BDS clause 18.1, as amended which shall be limited to Bid Securing Declaration or any of the following:
          1. 2% of the ABC for Cash
          2. 2% of the ABC for MC, CC, Bank Guarantee or Irrevocable Letter of Credit issued by a Universal or Commercial Bank
          3. 5% of the ABC for Surety Bond (The Surety Bond must be accompanied with a certification issued by the Insurance Commission and must unequivocally state that the issuing Surety or Insurance company is specifically authorized to issue Surety bonds callable on demand. It is also required that the certification must be project specific stating the details of the issued surety bond which includes the project name and bond amount.)

        1. Project Requirements, which shall include the following:
          1. Organizational chart for the contract to be bid
          2. List of contractor’s personnel to be assigned to the contract to be bid, with their complete qualification and experience data. The list of personnel shall be supported with the following:
            1. PRC License
            2. DPWH Certificate of Accreditation
            3. DOLE Certification
            4. Individual resume/bio-data

        1. List of contractor’s major equipment units, which are owned, leased, and/or under purchase agreements Supported with the following:
          1. Proof of ownership (O.R/C.R/Deed of absolute sale) for owned equipment
          2. Certification of Availability of Equipment from the Equipment Lessor/Vendor for the duration of the project as the case may be.

      1. Omnibus Sworn statement using the prescribed form.

SECOND ENVELOPE: FINANCIAL COMPONENT
  1. Financial Bid Form
  2. Bill of Quantities
  3. Detailed Cost Estimates for each pay item in the Bill of Quantities. The Direct Cost shall show itemized computation of materials, labor, equipment rental, fuel and oil. The Indirect Cost such as overhead/contingencies/miscellaneous, profit and the applicable taxes as required by law shall also be shown in coming up with the bid. It shall show detailed breakdown of LUMPSUM Cost.
  4. Price lists
  5. Cash Flow by Quarter and Payment Schedule

Failure to submit the above requirements or a finding against the veracity of such shall be ground for the forfeiture of the Bid Security and disqualification of the bid.
