Account holder may request for the reconnection of disconnected water service provided that certain procedures and requirements had been complied with by the account holder.

The general public within the service are of Surallah Water District


REQUIREMENTS

  1. Reconnection Fee:
    1. For disconnected within 15 days:
      1. Reconnection Fee of Php 200.00
      2. Full payment of arrears, if any
    2. For disconnected more than 15 days but not over 90 days:
      1. Reconnection Fee of Php 500.00
      2. Full payment of arrears, if any
    3. For disconnected for 3 months and above:
      1. Reconnection Fee of Php 3,000.00
      2. Full payment of arrears, if any
  2. Valid ID with picture for identity verification purposes

SERVICE DURATION

  1. For disconnected within 15 days – 1 day and 15 minutes
  2. For disconnected more than 15 days but not over 90 days – 3 days and 15 minutes
  3. For disconnected for 3 months and above – 5 days and 15 minutes

CLIENT STEPS

  1. Fill out Service Request Form (SRF) at the Customer Service
  2. Submit filled out SRF together with valid ID with photo to Customer Service Assistant
  3. Present statement of account to cashier and pay the stated amount
  4. Present official receipt of payment to customer service assistant
  5. Wait for the SWD personnel to conduct reconnection of water service
  6. Sign and receive consumer’s copy of service request form
  7. End of transaction
