A RESOLUTION DECLARING CHEMICAL RESEARCH PRODUCTS AS THE SUPPLIER WITH POST QUALIFIED BID FOR THE PROCUREMENT OF FORTY (40) DRUMS OF CHLORINE GRANULES AND RECOMMENDING TO GM ANTONIO V. CANIESO FOR APPROVAL AND AWARD
WHEREAS, the Surallah Water District advertised its REQUEST FOR QUOTATION for the procurement of FORTY (40) DRUMS OF CHLORINE GRANULES AND – PURCHASE REQUEST NO. 21-09-152 and in conspicuous place at the premises of Surallah Water District continuously for 7 days.
WHEREAS, in response to the said advertisement (3) suppliers have submitted its quotation for the procurement of FORTY (40) DRUMS OF CHLORINE GRANULES.
WHEREAS, the CHEMICAL RESEARCH PRODUCTS bid proposal was found to be substantially complying as shown below:

WHEREAS, after POST QUALIFICATION EVALUATION REPORT the member of the BAC has declared to recommend award to CHEMICAL RESEARCH PRODUCTS as the post qualified bid for RFQ NO. 21-09-091 and recommending approval and award to GM ANTONIO V. CANIESO.
WHEREFORE, premises considered and upon motion duly seconded, the committee
RESOLVED, as it is hereby resolved to declare, as it is hereby declared and recommend award to CHEMICAL RESEARCH PRODUCTS as the post qualified bid for the procurement of FORTY (40) DRUMS OF CHLORINE GRANULES.
RESOLVED finally, the copy of this RESOLUTION to be furnished to concerned personnel for their information
WE HEREBY CERTIFY TO THE CORRECTNESS OF THE FOREGOING RESOLUTION AS DULY ADOPTED ON SEPTEMBER 29, 2021, 2PM AT THE BIDS AND AWARDS COMMITTEE OFFICE, SURALLAH WATER DISTRICT, SURALLAH, SOUTH COTABATO.
