A RESOLUTION DECLARING CHEMICAL RESEARCH PRODUCTS. AS THE SUPPLIER WITH MOST RESPONSIVE BID FOR THE PROCUREMENT OF SUPPLY AND DELIVERY OF FORTY (40) BOXES OF CHLORINE DIOXIDE POWDER AND RECOMMENDING TO GM ANTONIO V. CANIESO FOR APPROVAL AND AWARD
WHEREAS, the Surallah Water District advertised its REQUEST FOR QUOTATION through PHILGEPS the procurement of SUPPLY AND DELIVERY OF FORTY (40) BOXES OF CHLORINE DIOXIDE POWDER – PURCHASE REQUEST NO. 21-08-145 and in conspicuous place at the premises of Surallah Water District continuously for 7 days.
WHEREAS, in response to the said advertisement (5) supplier have submitted its quotation for the procurement of SUPPLY AND DELIVERY OF FORTY (40) BOXES OF CHLORINE DIOXIDE POWDER.
WHEREAS, the CHEMICAL RESEARCH PRODUCTS bid proposal was found to be substantially complying as shown below:

WHEREAS, after through deliberation the members of the BAC has declared to recommend award to CHEMICAL RESEARCH PRODUCTS as the Most Responsive Bid being the previous supplier considering its product good quality for RFQ NO. 21-08-085 and recommend approval and award to GM ANTONIO V. CANIESO.
WHEREFORE, premises considered and upon motion duly seconded, the committee
RESOLVED, as it is hereby resolved to declare, as it is hereby declared and recommend award to CHEMICAL RESEARCH PRODUCTS as the Most Responsive Bid for the procurement of SUPPLY AND DELIVERY OF FORTY (40) BOXES OF CHLORINE DIOXIDE POWDER.
RESOLVED finally, the copy of this RESOLUTION to be furnished to concerned personnel for their information
